I’ve never logged onto the site before. What is my password?
Your login and password will be provided to you by your HR department. For team members who work at the following locations, your password will consist of: the first letter of your first name (capitalized), the first letter of your last name (lowercase), the word: Uniforms (capital U), and the last 4 digits of your social security number. Example: ScUniforms9999
Applicable locations: IU Health Arnett, Ball Memorial, Blackford, Bloomington, Methodist, North, Saxony, Tipton, University, West, White Memorial, and Riley Hospital at IU Health
If this is your first time logging in and you do not work at any of the locations listed above, please contact your HR department to obtain your password.

My uniform doesn’t fit. Can I return it?
Uniforms may be returned for any reason within 30 days prior to washing/wearing (tags attached). Sorry; no return of washed or worn uniforms will be accepted. Damaged or defective uniforms are eligible for return within 90 days. Please send all return items to: Shumksy, 2750 E River Rd, Moraine, OH 45439, along with a return/exchange form.
